目的:明确漏斗胸病儿营养状况和膳食摄入情况与漏斗胸严重程度之间的关系,为漏斗胸围术期营养支持和出院后营养指导提供科学依据。 方法:收集 2022年 1月至 2023年 3月在南京医科大学附属儿童医院心胸外科就诊并在临床营养科进行营养管理的 62例漏斗胸病儿的基本资料、实验室检查、影像学检查以及膳食分析情况。 结果:62例漏斗胸病儿中存在营养不良 39人(62.9%),存在心肺功能受损的病儿有 43例(69.4%),心肺功能受损组 BMI和 BAZ明显低于正常组(P < 0.05),两组间身高无明显差异。心肺功能受损组每日能量摄入百分比明显低于正常组(P < 0.05),Haller指数明显高于正常组(P < 0.05)。相关性分析发现能量摄入百分比与 Haller指数负相关(r = -0.314,P = 0.032),BAZ值与 Haller指数负相关(r = -0.3,P = 0.032),BAZ值与能量摄入百分比正相关(r=0.29,P = 0.04),差异有统计学意义。 结论:大部分就诊的漏斗胸病儿存在能量摄入不足和营养不良,畸形程度越严重对膳食摄入和营养状况的影响越大。
Abstract:
Objective: To clarify the relationship between the nutritional status and dietary intake of children withpectus excavatum and the severity of pectus excavatum, and to provide scientific basis for nutritional support duringperioperative period and nutritional guidance after discharge. Methods: The clinical features, laboratory examination,imaging examination and dietary analysis were collected from 62 children with pectus excavatum who were admitted tothe department of cardiothoracic surgery and the department of clinical nutrition, Children’s Hospital of Nanjing MedicalUniversity, from January 2022 to March 2023 were retrospectively analyzed. Results: Among the 62 children withpectus excavatum, 39 (62.9%) had malnutrition, and 43 (69.4%) had impaired cardiopulmonary function. The BMI andBAZ of the group with impaired cardiopulmonary function were significantly lower than those of the normal group (P <0.05), and there was no significant difference in height between the two groups. The percentage of daily energy intake inthe impaired cardiopulmonary function group was significantly lower than that in the normal group (P < 0.05), and theHaller index was significantly higher than that in the normal group (P < 0.05). Correlation analysis showed thatpercentage of energy intake was negatively correlated with Haller index (r = -0.314,P = 0.032), BAZ value wasnegatively correlated with Haller index (r = -0.3, P = 0.032) and positively correlated with percentage of energy intake(r = 0.29, P = 0.04), the difference was statistically significant. Conclusion: Most children with pectus excavatumpresent insufficient energy intake and malnutrition. The severity of the deformity affects dietary intake and nutritionalstatus.
