目的:探讨外周血炎症指标(包括淋巴细胞计数和C反应蛋白)和白蛋白,对顽固性便秘金陵术后发生肠道菌群紊乱的早期诊断价值。 方法:回顾性分析2019年9月至2021年9月在东部战区总医院诊断为顽固性便秘的住院病人,纳入符合标准的174例病人,分非肠道菌群紊乱组(159例)和肠道菌群紊乱组(15例),分析病人临床表现和发生肠道菌群紊乱的影响因素。采用ROC曲线评价外周血淋巴细胞计数(LYM)、C反应蛋白(CRP)和白蛋白(ALB)在肠道菌群紊乱中早期诊断价值。计算最佳截断值的敏感度和特异度。 结果:与非肠道菌群紊乱组比较,肠道菌群紊乱组术后CRP 明显增高(P < 0.01),而LYM和ALB显著减低(P < 0.01)。ROC曲线分析显示,术后LYM、CRP和ALB曲线下面积(AUC)分别为0.907、0.943和0.792。三者最佳截断值分别为0.62 × 109/L、105mg/L和33.0 g/L,敏感度和特异度分别为80.0%和91.2%、93.3%和81.1%、80.0%和76.1%。联合预测肠道菌群紊乱AUC为0.974。多因素 Logistic 回归分析显示,术后LYM ≤ 0.62×109/L、CRP > 105 mg/L和ALB ≤ 33.0 g/L是发生肠道菌群紊乱的独立危险因素(P < 0.05)。 结论:术后LYM、CRP和ALB是顽固性便秘金陵术后肠道菌群紊乱的独立影响因素,具有较好的早期诊断价值,三者联合诊断价值更高。
Abstract:
Objective: To investigate the early diagnostic value of peripheral blood inflammatory markers (including lymphocyte count and C-reactive protein) and albumin for postoperative intestinal flora disorder in refractory constipation following the Jinling surgical procedure. Methods: A retrospective analysis was conducted on inpatients diagnosed with refractory constipation at the Department of General Surgery of Jinling Hospital from September 2019 to September 2021. A total of 174 eligible patients were included, categorized into the non-intestinal flora disorder group (159 cases) and the intestinal flora disorder group (15 cases). Clinical presentations and factors influencing the occurrence of intestinal flora disorder were analyzed. Receiver Operating Characteristic (ROC) curves were used to assess the early diagnostic value of peripheral blood lymphocyte count (LYM), C-reactive protein (CRP), and albumin (ALB) in intestinal flora disorder. The sensitivity and specificity at the optimal cut-off value were also calculated. Results: Compared with the non-intestinal flora disorder group, postoperative CRP levels were significantly increased in the intestinal flora disorder group (P < 0.01), while lLYM and ALB were significantly decreased (P < 0.01). ROC curve analysis showed that the areas under the curve (AUC) for postoperative LYM, CRP, and ALB were 0.907, 0.943, and 0.792, respectively. The optimal cutoff values were determined to be 0.62×109/L for LYM, 105mg/L for CRP, and 33.0g/L for ALB, with sensitivities and specificities of 80.0% and 91.2%, 93.3% and 81.1%, 80.0% and 76.1%, respectively. The AUC for combined prediction of intestinal flora disorder was 0.974. Multivariate logistic regression analysis showed that postoperative LYM ≤ 0.62×109/L, CRP > 105mg/L, and ALB ≤ 33.0g/L were independent risk factors for the occurrence of intestinal showed (P < 0.05). Conclusion: Postoperative LYM, CRP, and ALB are independent influencing factors for intestinal flora disorder in refractory constipation following the Jinling surgical procedure, and have good early diagnosis value, and their combined diagnosis value is higher.
