摘要:
目的:探讨纤维蛋白胶对腹腔开放创面及腹腔脏器的影响。 方法:27只雄性(180 ± 10) g SD大鼠,随机分成三组:空白对照组(Ctrl组,n = 9),聚丙烯网片临时关腹组(PP组,n = 9),聚丙烯网片联合纤维蛋白胶组(FG组,n = 9)。于术后4、7、14 d分别观察造模后大鼠生命体征、体质量、创面愈合进度等;术后4、7、14 d后取腹壁和腹腔开放创面肉芽组织,行HE染色和天狼星红染色评价创面愈合情况。取腹腔脏器组织,评价各组腹腔开放后的脏器功能。 结果:腹腔开放造模后,在死亡率方面Ctrl组、PP组、FG组组间差异无统计学意义(P > 0.05)。与Ctrl 组大鼠相比,PP 组与 FG 组术后 4 d、7 d、14 d 体质量显著下降(P 值均 < 0.05),但 PP 组和 FG 组间无明显差异(P > 0.05)。与PP组相比,FG组术后创面肉眼可见更为整洁。术后14 d,FG组腹腔开放创面愈合率显著高于PP组(P < 0.05)。术后4 d PP组大鼠的肠道损伤程度显著高于FG组(P < 0.05),但是未观察到纤维蛋白胶对腹腔开放后大鼠肺、肝、肾的保护或损伤作用。HE染色结果显示FG组术后4 d和7 d增生肉芽组织厚度明显高于PP组(P <0.05)。天狼星红染色结果显示FG组的III型胶原蛋白浸润早于PP组。 结论:腹腔开放创面应用纤维蛋白胶保护,可促进肉芽组织增生和III型胶原提前浸润,并维持裸露肠管完整性。
Abstract:
Objective: To verify the protective effects of fibrin glue on wounds and intra-abdominal organs in rats with open abdomen. Methods: Twenty-seven healthy male SD rats were randomly divided into three groups: blank control group without any treatment; Polypropylene mesh (PP) group with polypropylene mesh temporarily closing the abdomen; Fibrin glue (FG) group with polypropylene mesh combined with fibrin glue; the abdominal cavity was opened according to the group. The general conditions of the rats were observed at 4, 7, and 14 days postoperatively, including vital signs, body weight, and wound healing. HE staining and histological score were also used to evaluate the damage to each organ after the abdominal cavity opening. Results: After the open abdominal cavity operation, there were no statistically significant differences among the Ctrl, PP, and FG groups in mortality (P > 0.05). Compared with the rats in the Ctrl group, the body weight of the PP and FG groups decreased significantly at 4, 7 and 14 days after surgery (all P values < 0.05), but there were no significant differences between the PP and FG groups (P > 0.05). Compared with the PP group, the postoperative wounds were more neatly observed by the naked eye in the FG group. At 14 days after operation, the healing rate of the open abdominal wound was significantly higher in the FG group than that in the PP group (P < 0.05). The degree of intestinal damage was significantly higher in the PP group than that in the FG group at 4 days postoperatively (P < 0.05), but no protective or damaging effect of fibrin glue on the lung, liver and kidney of rats after operation was observed. HE staining showed that the thickness of hyperplastic granulation tissue was significantly higher in the FG group than in the PP group at 4 and 7 days postoperatively (P < 0.05). Sirius red staining results showed that type III collagen infiltration was earlier in the FG group than that in the PP group. Conclusion: Fibrin glue can be used to protect open abdominal wounds from promoting granulation tissue proliferation and early type III collagen infiltration and to maintain the integrity of the naked intestine.
