目的:调查急性淋巴细胞白血病(ALL)病儿化疗诱导缓解期饮食摄入及营养状况,分析营养干预的重要性, 为临床开展营养支持提供依据。 方法:以 2018年 3月至 2020年 3月南京医科大学附属儿童医院行诱导 缓解方案的 87例新发 ALL病儿为研究对象,分别于诱导缓解期治疗的第 1天、第 15天、第 28天监测病儿的身高、体质量,动态评估病儿的营养状况,同时对病儿饮食摄入情况进行膳食分析。 结果:诱导缓解治疗第 1天 ALL病儿每日总能量以及三大宏量营养素摄入值分别为:总能量(225.91 ± 104.68)kJ/d,蛋白质(30.92 ± 17.60)g/d,脂肪 (28.54 ± 14.77)g/d,碳水化合物(156.79 ± 78.77)g/d。随着诱导缓解化疗的进行,至第 15天病儿各项摄入均明显减少(P均 ≤ 0.001)。ALL病儿诱导缓解治疗第 1天营养不良发生率为 11.5%,随着诱导缓解治疗进行体质量、中上臂围、肱三头肌皮褶厚度、白蛋白、前白蛋白等各项指标与第 1天相比较均明显减少(P < 0.05),第 28天营养不良发生率为 21.8%,营养不良发生率随治疗时间增加。 结论:ALL病儿初诊时即存在营养摄入不足,诱导缓解治疗过程中营养状况恶化,因此治疗期间需动态监测 ALL病儿营养状况,必要时进行个体化营养干预,以改善预后及病儿的长期生存质量。
Abstract:
Objective: This study aimed to investigate the nutritional status and dietary intake of children newly diagnosed with acute lymphoblastic leukemia and the changes during induction therapy, so as to assess the importance of nutritional interventions. Method: 87 children with acute lymphoblastic leukemia were enrolled who received induction therapy in Nanjing Medical University Affiliated Children's Hospital from March 2018 to March 2020. We assessed nutritional status and conducted dietary intake of the children on the first, 15th, and 28th day of induced remission treatment. Results: On the first day of induced remission treatment, the level of energy intake was (225.91 ±104.68) kJ/d, protein intake was (30.92 ± 17.60) g/d, fat intake was (28.54 ± 14.77) g/d, and carbohydrate intake was(156.79 ± 78.77)g/d. On the 15th day, the oral nutrient intakes significantly decreased (all P ≤ 0.001). On the first day of induced remission treatment, 11.5% of the patients were malnourished, with the induced remission treatment, the weight, triceps skin fold thickness, mid-upper arm circumference, serum albumin and prealbumin of the patients all significantly decreased (all P≤0.05). Conclusions: ALL patients exhibit a marked reduction in nutrient intakes before the induced remission treatment, the rapid increase of malnutrition after the start of treatment. Therefore, longitudinal nutritional monitoring and individual interventions are needed to prevent poor outcomes and adverse late effects in this population.
