摘要:
目的:探讨骨质疏松性椎体压缩骨折(OVCFs)病人入院时的营养状态与经皮椎体成形术(PVP)后的早期腰背部疼痛改善及功能恢复的相关性。 方法:选取安徽省亳州市人民医院骨科在2014年6月至2019年3月期间就诊的131例OVCFs病人。131例病人的营养状态评估均在入院24 h进行,采用微型营养评定法(MNA-SF)评估。在入院时,术后(1 w、6 w、12 w)时,采用VAS及ODI评估腰背部疼痛及功能。 结果:131例病人中,营养正常27例(20.6%),营养不良风险41例(31.3%),营养不良63例(48.1%)。术后(1 w、6 w、12 w),营养不良病人的VAS及ODI高于营养正常病人(P < 0.05)。术后1 w,营养不良病人的VAS高于营养不良风险病人(P < 0.05);术后6 w、12 w,营养不良病人的ODI高于营养不良风险病人(P < 0.05)。术后6 w、12 w,营养不良风险病人VAS高于营养正常病人(P < 0.05)。营养状态是术后(1 w、6 w、12 w)VAS及ODI的独立影响因素(P < 0.05)。 结论:OVCFs病人的营养状态越差,PVP术后早期腰背部疼痛越明显且功能恢复越差。
Abstract:
Objective: To investigate the correlation between nutritional status of patients with osteoporotic vertebral compression fractures (OVCFs) at admission and early improvement of back pain and functional recovery after percutaneous vertebroplasty (PVP). Methods: 131 patients with OVCFs were selected from June 2014 to March 2019. The nutritional status of 131 patients was assessed 24 hours after admission. The nutritional status was assessed by micronutrient assessment (MNA-SF). At admission, VAS and ODI were used to evaluate lumbar and back pain and function at 1, 6 and 12 weeks after operation. Results: Of the 131 patients, 27 (20.6%) had normal nutrition, 41 (31.3%) had malnutrition risk and 63 (48.1%) had malnutrition. After operation (1 w, 6 w, 12 w), the VAS and ODI of malnutrition patients were higher than those of normal nutrition patients (P < 0.05). At 1w postoperatively, the VAS of malnutrition patients was higher than that of malnutrition risk patients (P < 0.05); at 6 w and 12 w postoperatively, the ODI of malnutrition patients was higher than that of malnutrition risk patients (P < 0.05). At 6w and 12 w after operation,the VAS of malnutrition risk patients was higher than that of normal nutrition patients (P < 0.05). Nutritional status was the independent influencing factor of VAS and ODI after operation (1 w, 6 w, 12 w) (P < 0.05). Conclusion: The worse the nutritional status of patients with OVCFs, the more obvious the early low back pain and the worse the functional recovery after PVP
