目的:探讨使用胰酶联合 NaHCO3溶解液在两种温度下对 ICU病人鼻肠管堵塞冲管的临床应用效果。 方法:选取 2018年 1月~12月在 ICU留置鼻肠管需要持续输注肠内营养液,而出现管道堵塞情况的病人。1月~6月共 47例作为对照组,7月~12月共 53例作为实验组。采用负压原理以及小容量脉冲式方法,对照组利用胰酶 NaHCO3溶液在常温下进行冲管;实验组采用胰酶 NaHCO3溶液在 40度温开水复温 5 min后再进行冲管。观察比较两组病人在鼻肠管复通的成功率,消耗时间不同。 结果:对照组 47例导管堵塞中,管道复通成功并且继续使用的为 39例,平均耗时(209.18 ± 45.27)min,8例无法复通拔除管道重新予置管;实验组共 53例,复通成功的 50例,平均耗时 (83.76 ± 26.37)min ,3例复通失败,重新置管。实验组的管道复通成功率与对照组无统计学意义(P = 0.07),时间消耗明显低于常规组(P < 0.001),有临床意义。 结论:在 40 度温开水复温后的胰酶联合 NaHCO3 溶解液相比较常温下的溶解液,能够更加快速的复通堵塞的鼻肠管,但是在复通的成功率上两者无差异。
Abstract:
Objective: To investigate the use of trypsin enzyme solution containing sodium bicarbonate (trypsinNaHCO3 solution) at different temperatures in the obstruction of nasojejunal feeding tubes in ICU patients with enteral nutrition. Methods: A total of 100 patients who received enteral nutrient solutions in the ICU between January and December, 2018 were enrolled in this study. Of the patients, 47 cases were used as control group, and 53 cases as experimental group. The control group was treated by the negative pressure pulse flushing with the trypsinNaHCO3 solution at room temperature. The experimental group was treated by the same method with the trypsinNaHCO3 solution after 5-min incubation at 40℃. Results: In the experimental group, the obstruction of the nasojejunal feeding tubes in 50 cases (94.3%) were removed, which took an average time of (83.76 ± 26.37) min. The tubes in the remaining 3 cases were replaced. In the control group, 39 cases (83.0%) were successfully recovered. It took an average time of (209.18 ± 45.27) min, which was significantly more than in experimental group (P < 0.001). Basing on the data, the success rate of reconnection in the obstructed feeding tubes was higher in the experimental group than in the control group, and the time consumption was significantly lower in the former. The findings indicate that a 5-min incubation at 40℃ could improve the efficacy of the trypsin-NaHCO3 solution for removing the obstruction of the nasojejunal feeding tubes in EN patients. Conclusion: Compared with the solubilized solution at room temperature, the solubilized solution of pancreatic enzyme combined with NaHCO3 after reheating of boiled water at 40 degrees Celsius could quickly reconnect the blocked nasointestinal tube, but there was no difference in the success rate of reconnecting the two.
